- You've made serious, structured attempts and nothing shifts.
- Compulsivity extends beyond porn — spending, gambling, other sexual behaviour, substances.
- Depression or anxiety is in the picture and doesn't lift when the behaviour does.
- The behaviour is putting your job, your relationship, or your legal standing at risk.
- Shame is severe. If slips produce thoughts of not wanting to be here, that's a today thing, not a later thing.
- There's trauma underneath it. This is common and it doesn't get resolved by habit tools.
What to ask for: a therapist experienced with compulsive sexual behaviour, ideally working in CBT or ACT — those are where the treatment evidence sits. Be wary of anyone selling an expensive residential "sex addiction" programme built on a model the DSM doesn't recognise and researchers actively dispute; the field has commercial actors with a stake in the addiction framing.
